1. Oorzaken

- mechanismen
- re-entry cicrcuit in rechter atrium
- atriaal ritme 250- 350
- vaak geassocieerd met
- onderliggend cardiaal lijden:
- Myocard (myocardischaemie, kleplijden, myocarditis, cardiomyopathie, postoperatief cardiochirurgie
- congestief hartfalen
- longen (longen, longziekten)
- electrolietenstoornissen
- thyrotoxicose
- bij kinderen
- bij de meeste kinderen is er een onderliggend hartlijden

- kleine kinderen verdragen dit slecht.  Het kan leiden naar shock en/of congestief hartfalen.  Perinataal komt het wel voor zonder onderliggend hartlijden en spontaan verdwijnend na de neonatale periode.

 

2. Kliniek

- snelle regelmatige pols
- vaak tijdelijk: duur van seconden tot uren.  Vaak overgang naar sinusritme of voorkamerfibrillatie
- bij stabiele patient:
- palpitaties
- dyspnee
- zwakte
- syncope
- bij instabiele patient:
- hypotensie
- persistente angor
- longoedeem

- pols > 150/min

 

3. Diagnostiek

- ECG
- P-toppen: snel en regelmatig ritme van 250 - 350 /min
                 zaagtandprofiel in afleiding II, III en AVF
- AV-blok: meestal een 2:1 blok
               soms onregelmatig
- soms is eerst vertraging nodig voor de diagnose: door vagale manoeuvres of Adenosine 3-6- 12-12 mg snel IV elke 2 min
- Labo:
- CBC
- Elektrolyten
- Hartenzymen
- schildklierfunctie
- Rx thorax
- cardiomegalie
- verbreed linker atrium
- hartfalen
- Transoesofagale echocardio (best alvorens cardioversie)

- IV-lijn
- O2
- monotoring
- onstabiele patient
- cardioversie:
- liefst met sedatie
- synchroon
- eerst 50J
- dan 100, 200, 300, 360J

Is de patient stabiel?
-Indien neen:
cardioversie:(liefst met sedatie, synchroon)
eerst 50J, dan 100 - 200 - 300 - 360J
- Indien Ja:
1. volwassene:
- electrische cardioversie
- nog steeds de veiligste manier
- synchroon
- eerst 50J
- dan 100 - 200 - 300 - 360 J
=>anticoagulatie indien stabiele patient én indien risico voor embolen (flutter > 48u, overspringend flutter- fibrillatie, kleplijden, linker ventrikeldysfunctie, CVA in voorgeschiedenis)
- Medicatie
- ritmecontrole
- indien nog een goede hartfunctie (EF>40%)
- calciumantagonisten:
- voorkeur diltiazem (Tildiem): blus over 2 min van 0,25mg/kg IV. Zo nodig na 15 min 0,35 mg/kg IV over 2 min
- 2e keuze Verapamil: risico = hypotensie.  Posologie 2,5 - 5 mg IV in bolus over 2 min.  Nadien zo nodig na 15 - 30 min 5-10 mg IV, tot maximum 20 mg
- Beta blokkers:
- sotalol (Sotalex): cave nierinsufficientie.  Risoco torsade de pointes, dus goede monitoring bij opstarten.
- Metoprolol (Lopressor, Selozok), 100 - 300 mg/d in 1-2 giften PO
- Propanolol (Inderal, Propanolol), 80-  240 mg/d in 2-4 giften PO
- Digoxine:
- vooral bij chronisch hartfalen
- bolus 0,5 mg IV, dan 0,25 mg iv elke 4u tot gewenste effect.  Nadien overstappen op PO
- Indien verminderde hartfunctie (EF < 40%)
- Digoxine (vooral bij chronisch hartfalen)
- bolus 0,5 mg IV, dan 0,25mg elke 4u tot gewenste effect.  Daarna overstappen op PO
- anti-aritmicum klasse 3: amiodarone (cordarone)
- eerst bolus van 2 ampulles, opgelost in 20 cc glucose.  300mg/ 5-15 min.  Nadien het eerste uur 2 amp (300mg) over 1 uur.  Dan verder 6 amp/24u (900mg)
- calciumantagonisten: diltiazem (Tildiem)
- Bolus van 0,25mg/kg IV over 2 min.  Nadien zo nodig na 15 min 0,35mg/kg IV over 2 min
- GEEN Beta-blokkers!!!
- WPW-syndroom: zie farmacoconversie
- Farmaco-conversie
- indien goede hartfunctie (EF >40%)
- Anti-aritmica klasse 1
- Propafenon (Rythmonorm): posologie IV 1-  2 mg/kg tegen 10mg/min. 1 amp = 70mg/20ml.  PO: 450 - 600 mg/d in 2-3 giften.  1co = 150, 225 of 300mg
- Flecainide (Tambocor): posologie IV: 2 mg/kg tegen 10mg/min. 1 amp = 150mg/15ml.  PO: 2X100mg/d
- Anti-aritmica klasse 3
- Amiodarone (Cordarone): eerst bolus van 2 amp in 20CC glucose.  300mg/5-15 min. Nadien eerste uur 2 amp (300mg)/uur.  De verdere 24u 6 amp/24u (900mg)
- Procainamide
- posologie: infuus 0,2 - 0,5 mg/kg/min.
- NIET in het compendium
- Indien verminderde hartfunctie (EF< 40%)
- electrocardioversie
- anti aritmicum klasse 3
- Amidodarone (Cordarone): eerst bolus van 2 amp in 20CC glucose.  300mg/5-15 min. Nadien eerste uur 2 amp (300mg)/uur.  De verdere 24u 6 amp/24u (900mg)
- WPW-syndroom - met goede linker ventrikelfunctie:
- anti-aritmica klasse 3:
- Amidodarone (Cordarone): eerst bolus van 2 amp in 20CC glucose.  300mg/5-15 min. Nadien eerste uur 2 amp (300mg)/uur.  De verdere 24u 6 amp/24u (900mg)
- anti-aritmica klasse 1:
- Propafenon (Rythmonorm): posologie IV 1-  2 mg/kg tegen 10mg/min. 1 amp = 70mg/20ml.  PO: 450 - 600 mg/d in 2-3 giften.  1co = 150, 225 of 300mg
- Flecainide (Tambocor): posologie IV: 2 mg/kg tegen 10mg/min. 1 amp = 150mg/15ml.  PO: 2X100mg/d
Sotalol (Sotalex)
- cave nierinsufficientie
- risico torsades de pointes. (goede monitoring bij opstart!!)
- posologie: 2x/d 80 - 160mg PO.  1co = 160mg
- is een anti-aritmicum én beta-blokker
- WPW-syndroom met verminderde linker ventrikelfunctie
- electrocardioversie
- anti-aritmicum klasse 3
- Amidodarone (Cordarone): eerst bolus van 2 amp in 20CC glucose.  300mg/5-15 min. Nadien eerste uur 2 amp (300mg)/uur.  De verdere 24u 6 amp/24u (900mg)
- opgelet!:
-  vermijdt volgende producten: calciumantagonisten, betablokkers en digoxine!!
2.  kinderen
- eerste keuze:
- cardioversie (vooral neonati)
- digoxine
- alternatieven:
- Beta blokkers
- Anti-aritmica klasse 3
- Amiodarone ( Cordarone): eerst bolus van 2 amp in 20CC glucose.  300mg/5-15 min. Nadien eerste uur 2 amp (300mg)/uur.  De verdere 24u 6 amp/24u (900mg)
- Anti-aritmica klasse 1
- procainamide
- Quinidine

- bij jonge kinderen geen Verapamil!!! (Isoptine, Lodixal)
